ASP file upload and input form submission

Hi I can upload a file using two asp files, but am having trouble putting input screens on the form, ie upload this file using the browse button, input text = enter your name. On my second asp form it uploads the asp file and displays the filename but does not display the name if I use the request function.
Can someone help me.
