I ran your code and first of all you should put </form> right after <input type="submit" value="Send test for grading and posting">

example:

<input type="submit" value="Send test for grading and posting"></form>

However, I see what your trying to do and I'm sorry to tell you that what you want to happen will require a server side language to work the way you want it to.

I'm assuming you want to make a poll:

Another option is to use CGI to have the forms results automatically sent to you then you can enter this information into your site manually. It's easier but not the best solution.
