How i can make a voting or poling system

Hi,

Plz tell me ,How i can make a voting system means we will have a question and two or more options and user will vote on them then we have to calculate how many person or user have given vote to particular option.Plz help me ,i am in deep trouble.

plz tell me the source code of it.
