HTMLdoc problems - Need to convert HTML to PDF! Help!

I am trying to convert a simple HTML page to PDF using HTMLDoc, but it keeps outputting an empty PDF file. I am running this as a CGI/PL file off a hosting server, so I cannot add modules and such. I am testing in Windows using Perl locally (installed from ActiveState). What am I doing wrong? Is there an easier way to convert HTML to PDF? I have a form users fill out, then it puts the results into a new page with the images of the original PDF, then its supposed to put it back together as a PDF and ship it off to the site admin...Jeez this is a lot more complicated than I thought.
