Input type=radio type problem

I made a script and I'm having some errors using the input in an "if" situation. I'll post the code and if anyone would paste it into notepad as an html and run it and help me debug it... I would GREATLY appreciate it.

leave the default radio switch on, change A and D to forum IMG tags, for B just put "test" minus quotes, and leave C as it is. Leave 1 as it is, put in "5" minus quotes for 2, leave 3 unchecked, and 4 blank.

Itll open a blank popup because the error kept it from getting to the document.write line, so you can close the popup and the error msg will come up.

Sorry if I'm being too vauge, I just hope this post has enough info for you guys to help me :D
