Submit form sending the contents to email & attaching file to form

I have a form that I would like users to be able to add an image file to send to me. I can't find how to do this anywhere.

Also, how do I get it to send the results to my email? (preferably without CGI)

You can not get attachements with html for attachements you will have to use server side languages...jsp..php etc
