I know this will be elementary to most if not all of you but, I'm having trouble with the form on this page.

Through some phone calls with my ISP, I've managed to get it to be processed by their form processor but, the only info I'm getting in the emial being sent to me is a simple "Thank You". I'm not getting any of the details written by the site visitor in my emial.
What have I done wrong. My guess is that I should have more hidden fields in the code to tell their processor what to do with the info it sends.
But, then again, I've never had much to do with websites until about a month ago. I have no idea what I'm doing and I really need some help.
1. How do I get the info the user types into the contact form? ie name,comments etc.
2. How do I get it to send the info to two seperate email addresses?
3. Is it possible to replace the ISPs "Thank you for completing our form" page with my own seperate page that bears that message?
