Error on page??

For some reason my browser says there is an error on my page at http://www.surgimark.com/products/checkout.htm

My scripts all work correctly, but I just wondered why does it say error after you push the submit button at the bottom?
(make sure you leave a required field blank, then push place order and you will see the error on the left bottom part of the browser)

can anyone help me get rid of that?
