Multi choice calculating mail-in form code needed

I am doing a mail-in order form for a client which contains a fair number of items, each with a choice of various sizes and prices. If anyone has any idea where I can obtain a form script that allows the client to enter in a number of items in each choice and the total value of the order as the result, I would appreciate hearing from you. thanks
