My .php page not showing when published, but OK when previewing locally

I do not see the registration form on my .php page after I have uploaded this page to the server. But everything looks just fine I preview in locally in the browser.

Please help me find the error (or whatever) that prevents the page from showing. Here is the code:

I think you need to remove the very last quote, the one before the semicolon

VALUES ('".$_POST['username']."', '".$_POST['pass']."')";
