Problems with formmail.pl

I am having problems with a perl file called formmail.pl i am trying to configure it correctly so that it works on my host server but for some reason it is not processing the form data nor sending it to any place??? I would highly appreciate it if someone can take a look at the formmail files I am trying to configure and help me resolve this issue. THANKS!!!
