"Rate this song" feedback

I don't know where to post this... PHP could work, I guess.

I'm a songwriter. I want to make a page for each song I post on my site and let people give me feedback. Like, maybe there would be several questions and they would "rate" their answer from 1 to 10, 10 being perfect. Maybe some yes / no / sometimes answer choices. Maybe also a section for "general comments."

Know of any script / code available? I don't do PHP programming.

EDIT: I found this survey thingie (http://quibblo.com/create/survey) that could work but maybe a script would be easier. Once set up, I'd like to just copy and paste it for each song.

This is weird! I was going to edit my post but there is no Edit button there! (Hmmm, once I posted THIS post, there is an edit button for THIS post only. Maybe the Edit button disappears after awhile. Odd.)

Regarding your post, I should have been more clear that I am not looking for a simple "rate this song" type thing. I am looking for something where they'd score various aspects of the song from, say 1 to 10... Melody, story, arrangement, etc. Maybe some multiple choice answers and the other things I described.

Using that Quibblo interface, I created this survey (http://quibblo.com/quiz/1aAyBO/Rate-My-Song-C-As-Country-Store). I would prefer to have something I control. As mentioned, I'd like to have a "comment" section and maybe some other things.
