Help with password page

Newbie here! I am hoping to find someone that might be able to help me get my password page to work properly. I'm sure it is an easy task for someone that knows what they are doing. The site is www.itsstone.com click on price list

You'd be better of using serverside coding (PHP, Perl, etc...) to do this.
