ASP Form Send

I am having problem with some ASP code in a form. There are 3 pages involved. The first page is the info input, then it posts to another page which allows the user to confirm the info. then the send page. The first 2 work. The send page keeps throwing a 500 error when i try and send the confirmed data via email. Please help. See code below. Thanks.
