Turning existing graphic/page into a mail form

Hello,
I have created a 'contact us' graphic page http://www.sitenoticed.com/contact.html and I have created the page as a graphic as a whole. How do I program the "name, email, phone, etc" so I can have a working form? I have used formmail before but don't know if I should have created the form only first and then put that into the entire graphic and merge the graphics. Do I need to redo the entire graphic? I have Adobe CS3 Professional. Any help is greatly appreciated!!! Thank you:

if you want to keep the look and feel of that then you should simply make it as a form. you can set the backgrounds of the forms to the gradients you have already created, as well as the buttons to look how you want using styles.
