Help with dreamweaver forms and e-mail please

Hi,
I have only used DW a few times and I'm stuck on how to make a form send an e-mail to the owner's e-mail address. I want it to look like:
Subject:__________________
E-mail content:______________
______________
______________
From {sender's e-mail address}:___________
SEND CLEAR

Any ideas on one that would work?
