Looking For A Script Like This

I am putting together a site and the client wants a page with current trail conditions of the local area. I thought on using a comment based PHP script but did not like the ones I came across. Then I saw this one (http://www.crosscountryalaska.org/trails/index.php?trail_id=7) and that is exactly what I am after. However I can seem to find anything similar to it anywhere.

Can some one please guide me in the proper direction?

You can change it up a bit, but it will give you a general idea of how to do it.
