How was this image created

How would they have created the following image in photoshop. It is a rectangle which slides up to another level.

I know we have our basic rectangle, rounded rectangle, eclipse, polygon, line and custom tool BUT how do you create the image above

If someone has a different way of doing it than someone else please post, thanks in advance

you would have a rounded rectangle then add anchor points with the pen tool and move them. search for banners or headers at tutorialized.com I know there are some tutorials for that on there.

You could start with a rounded rect as a base and then add other rounded rects that subtract from the first one. But then it'd be a manual tweaking of anchor points to finish it out.
