Information from FORM to pre typed Contract

I know how to set up a form, and I have a third party client to action the form.

Each of the elements have a unique name as required.

What I am trying to do, is take the information entered on the form, and transfer each element to a specific place on a pre written contract.
Once done, get the complete contract emailed as an attachment to the person who filled in the form.

eg Fields on Form
Name (name)
Address (address)
Phone (phone)
Email (email)

Contract
I (name) confirm that my address is (address) and my phone number is (phone)

The above is a short example of what I am trying to achieve.
The actual form is a lot longer with several more elements.

Is all this at all possible please?

Yes, it's possible.

Post the form to a server side form handler that validates your form input, then creates your e-mail text and a MIME e-mail.
