Refer-a-friend script

Anyone know of a simple refer-a-friend script? Where they refer a friend to your sitename... fill in a simple form on your site with the friend's email address. Doesn't have to be php, I don't care.

well if you want to use PHP, have a form with an email field, have the form post to a page with a mail() function, http://php.net/manual/en/function.mail.php
