help finding the full script

This is a rate and review script. Any help with this would be greatful! It includes::
rate.cgi
birdcast2.cgi?

I've seen this script on quite a few websites but I don't know where it came from and I need it. Here is an example of how it's used:

lol i tried the google search and webcrawler and still couldn't find it. i think the original cgi script has been modified... but the website was interesting to read. :)
