How can I require a user to fill out and send an info form prior to viewing or downloading a PDF catalog?

just have a link on where it says d/l here or whatever taking you to the form page, on submitting the form (which should be validated) it takes you to the d/l page which should be masked (not display the address) so other users cant bypass system.
