Forms Help!

Hello! I want to create a checkbox form that a user can fill out and submit. Then the user will automatically receive an email with info about each selected item. Or a page will be generated containing all the selected info and they can print it out. Is that possible/difficult? Any tips? Thanks!

Did you have a specific question or just want someone to build the program for you? If you just want a premade package, there are plenty to choose from on the net.

I don't know what I should do at this point. I would really like for someone to build it for me--or at least give me a template exactly like what I need so I can input my information. I have been searching and reading for days and I haven't found anything that helps. Thanks!
