I put in vbforums.com, along with the primary keyword for that site which is Visual Basic, and I got an error, all fields are required. All fields were filled in. I tried another site, got the same message.

Yup, same here. I got the "all fields are required" message.
On the other hand, why are you hardcoding "www." into the domain name? what if it's a non-www website or has a redirect rule for the www-version?

Worked fine for me. Very nice tool, very responsive, actually the best one I've seen that does this. Consider it delcious'd. ;)
My one suggestion would be to get rid of the "clear result" button and simply make it automatically clear the previous result when you search for something new. I think it's safe to assume that people want to clear the old result when they do this.
An alternative would be to have the previous results in a list below... the last search gets added to the top of the list and maybe the list holds 10 max.

Nice, it worked for me. I like the fact that the clear button does not clear the whole form, because it was convenient to search for more than keyword for the same site. The only problem I have with it is that I refreshed the page and it became completely inoperable.
