Trouble inserting web counter!

This is crazy! I have done this before, but now I'm having no luck inserting a site counter into my web site. All I receive is the usual "small box with the red x". I have tried many things but now I'm drawing a blank. Feel free to tell me what Information you may need to try to help me correct this problem.

Did you move your site from being under the root domain to being an independent site with it's own cgi-bin? if yes, you'll need to correct the code.
What counter do you use?
