send email from html form

Hello,

I'm trying to figure out how to send an email to my personal email address with information that a user submits on a form. The page is here (http://www.themadisonproject.com/madproj3/events/book.html).

If anyone can suggest any useful resources/tutorials or sample code that might help me out, I would greatly appreciate it.
