Not Showing 404 Status Code

I've created a custom 404 error page that looks like my site. However, instead of returning a 404 status code in the header it's returning a 200 status code. How do I tell the page to use 404 instead of 200?

How are you assigning the 404 page? Is it through .htaccess or some other way?
