how use 2 destination forms?

hi,
i have a form with some inputbox inside of them
now i want creat 2 bottons each one with a destination url.

for example botton 1 will save the data and close the form
the botton 2 will save data and show the data that was saved

but how can i use the same controls with two diferent forms?
