I am currently recovering from a back injury and as a result I am on my back on the couch with my laptop all day long. I have been having a blast designing my website!

I had this idea that I could offer to design a simple website for my chiropractor in exchange for her services. I'm in the process of writing up a proposal.

My first question that I have right now is:

When people pay you for web design if you charge a flat rate does it usually include fees for hosting and domain name purchase?

As for me, I generally give a price that will cover my compensation for building the site, and let the customer know what they will be the annual costs for hosting/domain registration - I don't include these costs in my fee for the project, I have a routine host and registrar that I use and I say something like $XXXX for the website as discussed ('as discussed' can be a whole other ball game) and it will cost you $XX per year for the domain and $XX per year for hosting. Hope that helps.
