Looking for a professional, good looking online survey tool

I would like to offer the visitors of my company's website an online survey concerning visitor satisfaction. Till now I haven't found the right tool for my company, hopefully you can help me out.

The most critical demands for the survey tool I am looking for are:
- The tool is looking modern and professional
- A good looking question box asks the visitor (once) for a "Do you want to take this survey". Not a simple, old fashion HTML popup (popup killer), but a modern layer based, java or flash tool

So, actually I'm looking for a online, modern online survey tool.

Anyone can offer me some hints?

Ajax Poller (http://www.dhtmlgoodies.com/index.html?whichScript=ajax-poller) works well out of the box. There are a lot more options... depends on what you need and what you are comfortable with. Search for "poll scripts" adding other words like "php" or "ajax" or "flash" to find them.
