I believe you would have to use a Server Side Language for this. Such as PHP, ASP, CGI ect. ect.

yes. You would need the scripting to be done server side so it will be secure and you need to log the reasults in a data base. I am working on a poll right now. I am learning about dynamic images so I can have the stats and all.
