Questions for Phpbb forum pros

A few questions, guys:

My forums seem to have permalink like this: http://teamworkgo.com/forum/adm/index.php?i=forums&sid=d75fe392d................. (whatever) Instead i would like to see each page like this: http://teamworkgo.com/forum/category/ http://teamworkgo.com/forum/post-title/ Where do i change permalinks.

1) I need to add some mandatory field in signup form to prevent auto-spam (will it actually help?)
2) I need to change the logo though C-panel
3) I need to add a static banner / hyperlinked image to the major header and footer across all forums
4) I need to add custom header and footer (specified by admin) for each new post made by members, not major forum's header but just a message (above the member info and under his signature)
5) I need to add AdSense banner above members' signature in each post.

I am not a programmer and would really appreciate references to video tutorials or a very detailed description of which files to modify, where to place code, how the code would look like exactly (examples, not just pointers.)

I've never been to that problem like yours but I would be definitely want to know whats the solution for that problem. So that if ever I encountered these kind of problem I already know where to go.
