Step by step needed to sell/transfer a domain name

Is there a step by step publication of actions needed to do to transfer the ownership of a domain name?

I am looking to give up a name to a local school band program that we created a couple years ago.

Basically, most of the time, you should just move the domain to the other party's account. It would require them to open up one prior of making changes. Usually it is done free of charge within the same registrar (for example, it's called "Account change" in GoDaddy).
