Form - email address

I have a form created in frontpage2000. I have the form sent to me via email. However, anytime someone submits the fom, the "from" always states My email address instead of the person sending the form.

How do I fix this to have the "from" box actually state who completed the form

Thanks Dave. I contacted my ISP and they are re-installing Extensions. Hopefully that will do the trick.

On another issue, I have a javascript that has dropdown menues for reginos, then when someone select a region, they then select a country. When someone selects North America as a region, they get a choice of Canada or the US. If they select the US, they then can choose a state.

When I put this javascript into a FrontPage form, the script works but I cannot send the completed form as an email.

Any thoughts?

What I am trying to do is have the form results emailed to me in a text form.

On forms where I do not have this script, all results are received fine.

But when I put the script in the form, I do not receive any results and the email does not send.
