Sending form entries in e-mail

How do I make a form send the entries to me in an e-mail? Here is the form: http://www.theville.org/~stevo. How can I make it do this?

I tried using the second method, but it just sent me a blank e-mail. I'm not sure how to use a server-side code, though.

Stevo, you might find it helpful to know that servers often have some sort of mail handling script available. Talk to your server people to find out if this is true in your case and if not to find out what languages are available.
