GuestBook/Basic Message Board

Does anyone know where I can download a basic Message Board/Guestbook like the one here: http://www.sudburytoday.co.uk/mk4_guestbook/guestbook.aspx

I can host php/CGI stuff and have access to MySQL, but nothing too complicated to setup..
