adding an application form to my site.

i am trying to add an application form for people to fill in on the screen and then print out, but i am struggling.

Does anyone know where i can look for further help??

I would suggest using a cgi script that passes the information collected on your form to a results page made in HTML.
Something like an online Will Creator.
