CGI/Perl Guestbook Script Not Working

I'm trying to create a guestbook on my website, I'm using a CGI/Perl script and I can't get it to work. Does anyone know anything about Perl that can help me? You can view my site at www.sweetpeahavanese.com and click on the guestbook at the bottom of the left frame.
