User Online Web Poll

Dear all. I m nowadays in a problem that i have to put user online poll (5 answers option). Please, send the complete code and ya 1 more imp thing, when a user click on submit/vote/go button the new result window shall be open. I do not want result in the same frame. Do you have ASP or PHP and a database program, or CGI rights? Without these you cannot host the poll yourself, although plenty of sites provide ad-supported polls.

Dear, Our web server is competible with ASP / MySQL. But the thing is, I want to add a user poll on my 1st page, so after entering the option, user might get his/her/ answer on other (small) window. Is this required any database connectivity? I was expecting that JavaScript could do this easily withour any DB connectivity. Regards.
