How Can I Add Checkbox-Limiting Feature To Existing Form?

Greetings,

I found a great checkbox-limiting feature at

and would like to incorporate such a feature into an existing form I'm using on my website ... and of course have the results (the user-checked boxes) summed into the resulting email.

Trouble is, the feature I found is in javascript, and my current form isn't.

I'd appreciate any guidance.
