I need help on creating a form requesting each user to say what they thought about my Web site. If the user closes this l form window before submitting it, a new one should automatically be regenerated.

If I understand you correctly you want a window to open when someone leaves your site, with a form in it. If they close the window with that form in it, a new window will popup with the form on it?
