HELP! Will someone help or create a form for me?

Hi i can't code very well and i need a form that fits these qualifications:

1. anyone can send it off the site
2. must include their email
3. can only be sent to one person and that email cannot be edited.
4. it's a predetrimed subject and information this can not be edited...

Hmm.. I'm not exactly sure what you want... You want anyone to be able to click a button and it will send an e-mail telling you their e-mail address? And it has a message and subject that they can't change? Is that it? If that's what you want, let me know...

It sounds like you are looking for a custom solution... I'm sure many would be willing to do this if it were a contract programming job (myself included).
