please note that the title of your thread ('Need Help :)') isn't a very helpful title. Next time you post, please write a more meaningful title, for example: 'T-shirt printing', or 'T-shirt graphics', etc..

Now to your problem,
You'll need a graphic editor program like Adobe Photoshop (http://www.adobe.com/products/photoshop/main.html)
And/or you'll probably need to work with vector graphics for this kind of printing so i'd advice either Adobe Illustrator (http://www.adobe.com/products/illustrator/main.html) or some equivalent from Corel Draw, or such.

PaintPro is the 'cheaper' alternative to Photoshop, but it does probably 99% of the same things (although I think that PhotoShop's icons are a bit more 'intuitive'... I still get confused to functions and uses using PaintPro and I've used it for about 4 years now).

When you print 'for T-shirt transfers', you will need a special printing paper, available at places that sell computer printer paper, and stores like Wal-Mart, etc. has it too. Anyway, when you print 'for T-shirt transfer', there is a special setting in your printer's list of sizes and the one for "transfers" is what you want. It will 'mirror' the image, so that when printed on the transfer paper, it is 'backward'.
That way, when you iron it onto a shirt, it become 'right-ways'.

For those whom have used this before, my telling it probably seems redundant. But if you've never done this before, the first time or two you might not get it right. It takes practice, but once you've done it a few times, it's really alot of fun.
