seeking survey that formats and tabulates results

Anyone know a good resource for a survey script ... cgi preferable and I have no experience with databases or mysql ... I can install cgi tho ;)

Nope, not really. There was a silly one on BigNosedBird I think somewhere ... but I would prefer something more robust and maybe customizable. Haven't been looking recently, but now that this has come back up (thank you) I'd love to get back "on the case"
