Place the above in a blank file. note the lack of the closing tag, the function call operator '()' and the full opening tag '<?php'...

Looks like you have semi-colons after all your if statements? I'd delete them all and see where you are at.

That usually means there is white space before or after your php tags. I'd double check to make sure there is nothing at the beginning of the file or at the end of the file.
