I have a javascript that slides open a div which contains form elements. Within the div is a table that can dynamically add rows by clicking a button (more javascript).

The PSD is probably more of a mock-up of what the form would look like once it is built. You're going to have to build it or find someone to build it using html code.
Kor is right. You'll have to practically re-create it using the individual pieces from the InDesign file but since it is already laid out, it might not take too much time to do that.
