Comments box/thread

Hi,

How would I create a comments forms so that users can leave commnets. Similar to what you see on facebook. So far I have thought of creating a blog, but, because it has the comments option, but that is not what I am looking for. I am not looking to setup a forum either.

In fact I am asking users to answer a simple 1 question survey, after its completed they should be able to enter a comment and also read other users comments. What are my options please!
