Why does alert show 3 times?

When you click on the "Printer Friendly Order Summary" button, an alert for the req'd fields popups up THREE times and I don't know why.... it's also popping up EVEN when all req'd fields have been filled out, and then it proceeds to the next page as it should. This used to work, I have no idea what happened!

I meant to say that you need to click on the FAX/MAIL ORDER FORM BUTTON to get the screen with the form - that's the screen with the "Printer Friendly Order Summary" button...
