Need to add an mp4 with controls and adjustable size

I want to embed an mp4 into an HTML page. First, I want the video to have controls. For some reason, I am having trouble adding these. How do I do this? Also, I want the object to take the size of the video being played. "Auto" apparently doesn't work. How can I resize it?

Yeah, don't use an .mp4 because of browser plugin and QT Moov Atom issues. Use Flash .flv or if you must, f4v. Lots of Flash players available on the net, like

Much more viewer friendly.

I don't really have a choice in the matter. Ideally, I'd use Flash, but this is the file that I need to work with.
