Need Help on This

Need help on this project ... I'm a Coach and this season is coming up is softball/baseball and my ? is that in a few weeks we will be doing a fundraiser for the kids 5-17 and i need to know how to build/make this its a sign up form here's a image.

when they click on the sign up "red circle" it opens a window maybe like a pop-up they put there name and press enter and then the sign up changes to there name and a remove link to remove there name when done.
